Download and submit your claim form to [email protected]

For non-emergency claims, please call us to notify us of the claim before you engage any contractors. Then, download and complete the claim form with your particulars and full facts of the claim including its occurrence, detailed circumstances and extent of loss.

Submit your original documents

We strongly encourage you to submit your claim documents via email to [email protected] for faster processing. Submit the form with the following supporting documents as soon as reasonably possible, but no later than 7 days after the incident:

 

  • Police report if applicable.
  • Death certificate, autopsy report and coroner's findings for a death claim.
  • Proof of relationship between deceased and claimant for a death claim.
  • Medical report or discharge summary on onset date, cause, extent of permanent disability (if applicable) and nature of injury or illness.
  • A dog or cat pedigree certificate and confirmation from a qualified and registered veterinarian of the cause of the animals' death.
  • Letters confirming cost of replacement documents etc.
  • Original purchase receipts.
  • Loan or hire purchase agreements.
  • Contractor's invoice(s).

 

We will inform you if we require original documents.

We'll be in touch

You'll receive an acknowledgement email from us within 3 working days after your claims submission. If we need further information, we'll get in touch within 30 working days.  

 

Once your claim is approved, you'll receive an email from us and your claim payout will be paid via PayNow within 3 business days.

 

Please ensure that your PayNow account is registered under your NRIC in order to receive the payment successfully. Otherwise, the bank will mail you a physical cheque in 14 business days.
