Read these important notes before you begin

You only need to complete a single claim form even if you have multiple long-term care plans with us.

 

If you are currently unable to perform Activities of Daily Living (Washing, Dressing, Feeding, Toileting, Transferring and Mobility/Walking or Moving Around), please follow these 3 simple steps to make a claim:

Setp 3: Visit your Appointed Assessor

 

Please submit the necessary documents for your claim:

a) Severe Disability Claim Form - To be completed by Assured (Policyholder)
 

b) Letter of Undertaking and Indemnity to be completed (applicable for Third Party Payee or Nursing Homes/Medical Institutions only)
 

c) Copy of all diagnostic/laboratory reports, Inpatient Discharge Summary and any relevant hospital reports that are available (refer to the respective Doctor’s statement on the list of required reports)
 

d) Copy of Birth Certificate/Adoption Document of the child (applicable for Dependent Care Benefit only)
 

e) Copy of front and back of Identity Card or Passport of the Caregiver/Third-Party Payee (Digital NRIC is not acceptable)
 

f) Copy of bank passbook/statement with the bank account owner’s full name, account number and bank’s official header clearly indicated on the same page must be submitted as proof of account ownership (applicable for Homes/Institutions only).
 

g) Finalized Police report including any investigation notes
 

h) Any other documents that support the claim (e.g. official certificate of appointment of the legal guardian of minor beneficiary)

 

Mode of Payment

For better payment experience, SGD payments to the Payee will be credited to the bank account linked to the Payee’s PayNowNRIC/FIN. Please check that you have registered for PayNow with your bank, using your NRIC/FIN.

Make an appointment

Make an appointment with an Appointed Assessor for severe disability assessment.

Visit your Appointed Assessor

The assessor will submit the severe disability assessment to your insurer once it is completed.

 

For severe disability assessment, you may need to:

 

  • Pay S$100* if the assessment is done at the assessor's clinic

  • Pay S$250* if a house call is required

  • Bring any available medical records and/or Inpatient Discharge Summary that you have

  • Bring your medicine (if any)

 

*The revised assessment fees are effective from 31 January 2020.
